Male circumcision tied to lower HIV prevalence

Programs in eastern and southern Africa changing people?s views on the procedure

WASHINGTON ? An increase in male circumcision in a South African community coincides with lower overall HIV rates among adult men in that population, a new study finds. Meanwhile, a report from Kenya bolsters earlier findings that young men who get circumcised are less than half as likely as their uncircumcised peers to acquire HIV.

The two reports were presented July 24 at the 19th International AIDS Conference.

Researchers in Orange Farm, South Africa, devised a community-wide program in 2008 that provided free circumcision for men, plus condoms and counseling. Within three years, the adult male circumcision rate jumped from 17 percent to 54 percent, said Bertran Auvert, a physician at the University of Versailles in France who works on the project.

Surveys of several thousand men in Orange Farm in 2008 and 2011 found that the prevalence of HIV dropped from 15.4 to 12.3 percent during that time. Auvert calculated that there were 1,040 HIV infections avoided.

The decrease in HIV prevalence, he said, should lead to a decrease in HIV rates for the men?s female partners. ?It?s the beginning of the story, so we?ll need some time,? he said.

An earlier trial in Orange Farm was among the first to show that male circumcision can reduce HIV infection risk. Now, a new analysis of data from a similar trial in Kisumu, Kenya, suggests the protection is long-lasting. Epidemiologist Robert Bailey of the University of Illinois at Chicago reported that 66 months after circumcision, men who underwent the operation were still less than half as likely to become infected with HIV as men who didn?t undergo it.

?After five and half years of follow-up, the fact that these results are consistent over time suggests the protective effect is sustained,? Bailey said.

But the apparent protection provided by circumcision is partial, not total. The foreskin around the penis of an uncircumcised man provides a warm, moist environment that can harbor a virus ? and from which it can invade the body or subsequently be spread to a sex partner. Removing the foreskin by circumcision leaves toughened skin that?s less susceptible to viral infiltration and transmission, Bailey and others have theorized.

Researchers had noticed a link between circumcision and reduced HIV risk when maps of Africa showed higher HIV rates in regions, notably eastern and southern Africa, where male circumcision wasn?t performed as a traditional practice (SN: 1/3/09, p. 14). Before the first circumcision trials in Africa were started about a decade ago and results from those tests were made public, many people in Africa ?thought that circumcision equated to becoming a Muslim,? said Samson Kironde, a physician in Jinja, Uganda, at JSI Research & Training Institute and USAID. The community-wide effect seen at Orange Farm ?is significant,? he says, ?and that?s the aim.?

Kironde says that by some estimates, circumcising 80 percent of men in a community ?would allow you to reach a critical mass of people? that would have a potent community-wide effect.

Now there are hints that male circumcision is gaining steam in these regions as a lifestyle choice.

A survey of 101 Kenyan women whose male partners had been circumcised found that all the women were satisfied with the choice and that 91 percent reported enjoying sex more since the operation. Timothy Adipo of Nyanza Reproductive Health Society in Kisumu, however, reported that one-third of the women also said condom use was less necessary since circumcision. ?They perceived themselves at lower risk,? Adipo said. ?Our educational message needs prioritizing now.?

Compensation controversy over 'Adriatic Alcatraz'

GOLI OTOK, Croatia (AP) ? It rises up from the pristine waters off Croatia's coast, a forbidding mound of rock known as the Adriatic Alcatraz.

The amount former political prisoners are being offered for each day they spent on this hell: $9.

The paltry compensation Serbia's government is granting people imprisoned by late Yugoslav dictator Josip Broz Tito in one of Europe's most notorious post-World War II labor camps has triggered outrage among many ? and threats of a lawsuit.

"It's miserable," said Bozdar Vulovic, one of the few living survivors of the Goli Otok, or Barren Island, camp that was created in 1948.

Nearly 600 prisoners of all ex-Yugoslav nations ? Serbs, Croats, Bosnians, Slovenes, Macedonians and Montenegrins ? are estimated to have died on the island from torture and disease; thousands suffered psychological damage from an infamous prison system that pitted the prisoners against each other in a battle for survival.

Goli Otok housed up to 16,000 political prisoners over an eight-year period. They ended up there after being accused by Tito's regime of expressing sympathy toward Josef Stalin and his hardline Soviet dictatorship. Tito, who espoused a more liberal brand of communism, broke with Moscow in 1948, earning him support from the West.

Some of the former Serb inmates are threatening to take the government to an international human rights court for what they say is meager compensation for the horrors they went through.

Others say they will reluctantly accept the offer because they have no time to waste on lengthy legal procedures.

"If you were my age, and if you were in bad health, what would you do?" asked 91-year-old Smilja Filipcev, who spent more than three years at Goli Otok as a woman prisoner.

Serbia has now belatedly decided to rehabilitate the victims of Tito's communist regime, including ex-dissidents, royalists and the Goli Otok inmates.

Authorities said they arrived at the $9 a day figure because Croatia had offered a similar sum. But that was 10 years ago when the money was worth much more. Croatia also gave its nationals "privileged" pension status that resulted in a much higher total package than the Serb one.

In 2003, Slovenia offered its ex-prisoners ?6,300 for each year spent on the island. The other former Yugoslav republics ? Montenegro, Bosnia and Macedonia ? have not yet compensated their nationals.

"The fear that we went through is something that should be compensated," said Vulovic, who was incarcerated on the island for nearly three years. "And great fear it was,"

The controversy in Serbia has revived painful memories of the island camp kept secret for decades. Its existence was largely ignored in the West because inmates were thought to be hardline pro-Soviet communists, not dissidents like the ones in the ex-Soviet gulags in Siberia.

Tito was the undisputed leader of Yugoslavia for 35 years until his death in 1980. Those who publicly declared support after the split with Moscow were summarily dispatched to the island, without a trial.

Vulovic, an 85-year-old Serb citizen who was born in Montenegro, said the inmates on Goli Otok had arranged a "welcoming event" for him and the other newcomers.

"They screamed at us yelling 'down with the scum' and beat us with their hands," he said. "At Goli Otok, we had to spy on each other, and we had to beat other inmates up if they ordered us to do so."

"We were beaten all the time, while working, going to breakfast or to lunch," he said, adding that neither the International Red Cross nor any other international humanitarian organization ever visited the island while he was there.

Vulovic said trivial acts could land people in the camp ? which was often compared to San Francisco's Alcatraz island because no one ever managed to escape.

"It was enough for someone to listen to radio Moscow, or privately say something good about Stalin, to be reported to the secret police and end up on the island," Vulovic said at his small Belgrade apartment with Stalin's portrait and the Bible side-by-side on a bookshelf.

"I was arrested when I was 17," Vulovic said. "The reason for my arrest was stupid. Somebody had written 'Down with Tito, long live Stalin' in chalk on the walls all over my neighborhood" in the Montenegrin seaside town of Kotor.

"According to the logic of the time, they thought that only high school students had access to chalk. So they arrested six of us from that neighborhood, and they beat us until we had to confess that we were the ones that had written those words."

In 1956, the island ended its days as a political prison and was turned into a high-security facility for the hardest criminals. Years after Tito's death, Croatia's tourist organization tried to reopen the 1.9-square-mile (3-square-kilometer) island for tourists willing to pay to re-enact the life of a political prisoner ? including hard labor, stale food and nights in solitary confinement.

The plan, which even had the support of some former inmates who offered to work as tour guides, was scrapped because of the protests by Goli Otok political prisoner organizations.

"This is where the infamous Goli Otok lynches started," said former inmate Vladimir Bobinac, 85, while interviewed at the island, describing a lane of prisoners forced to organize welcoming party for new inmates.

"People were badly beaten, some even died," Bobinac said. "They were beaten out of fear. Those who did not deliver the punches were beaten themselves."

Filipcev, who retains an exceptionally clear-mind in her advanced age, said she was sent to Goli Otok from Serbia because she refused to denounce Stalin in his clash with Tito.

"They loaded us onto freight cars, and we were off," she said.

"When we arrived, they opened the car doors and we saw the sea in front of us. They kicked us out of the freight cars, and we waited for a boat to come pick us up. They loaded us into the ship's hull. They didn't wait for us to go down the stairs, they pushed us down, and we fell on top of each other. Some women fainted; some had their legs and arms broken."

She said that on the island, all the prisoners, including women, were forced to hard labor, mostly digging stones in a quarry.

"There we had wooden boards, with handles on each end, used for carrying heavy loads," she said. "You would trip and fall down onto the rocks. If you got injured, they'd make you sit down and break rocks until your injury was better, and then they would put you back to carrying rocks."

She said that when released, she came back home to Serbia.

"At the time when I was arrested, my elder daughter was four years old, and my younger daughter was two, so when they saw me, the first thing they asked was 'Are you really our mother?'"

Euro rises on ECB talk, earnings buoy Dow

NEW YORK (Reuters) - The euro rose against the dollar for the first time in six days on Wednesday after suggestions that European policymakers will consider new ways to tackle the region's debt crisis, while U.S. earnings supported blue-chip stocks.

Higher 2012 profit forecasts from big manufacturers Caterpillar and Boeing and an outlook for quarterly revenue growth from chipmaker Broadcom helped to offset news of an unexpected drop in new U.S. home sales.

On Wall Street, the Dow snapped a three-day losing streak and the S&P and Nasdaq slipped only modestly, even as shares of Apple Inc, the world's most valuable publicly traded company, fell 4.3 percent to $574.97 after it posted a rare miss in revenue late on Tuesday.

"I'm encouraged investors are taking the Apple news pretty well," said Jack Ablin, chief investment officer at Harris Private Bank in Chicago.

Ablin said after a more than 40 percent rise in Apple shares this year, Wednesday's drop is but a speed bump, adding that the raised outlooks from Dow components Caterpillar and Boeing "is a huge positive for the market."

The Dow Jones industrial average rose 58.73 points, or 0.47 percent, to 12,676.05. The S&P 500 Index dipped 0.42 point, or 0.03 percent, to 1,337.89. The Nasdaq Composite Index fell 8.75 points, or 0.31 percent, to 2,854.24.

The euro rose 0.8 percent to $1.2156, although the outlook remains weak and it remained only just above a two-year low of $1.2042 hit Tuesday.

A member of the European Central Bank's Governing Council, Ewald Nowotny, said there were arguments for giving Europe's new permanent rescue fund a banking license, enabling it to borrow unlimited central bank money and boosting its capacity to prevent the euro zone debt crisis from spreading.

Analysts said the market may have put too much emphasis on the comments, given other ECB officials' opposition to the idea. Investors would likely sell into the euro's rally, the analysts said.

"The market is grasping for any positive news," said Omer Esiner, chief market analyst at Commonwealth Foreign Exchange in Washington, D.C. "It adds to the already somewhat improved tone. Whether or not it lasts is the question."

Market support from central banks has been expected for weeks as economic data sags globally. Weak readings on Britain's gross domestic product and on a German business sentiment survey added to worries about slowing growth.

FED TO ACT SOON?

Top Fed officials recently spelled out what measures they might take to boost growth and hiring. Fed action could come as soon as its policy meeting next week, on July 31-August 1.

"Right now in my view it's still Fed rhetoric, but enough to keep investors confident to take risk. I don't necessarily see tangible action. I think the Fed is afraid to take action, fearful the market could shrug it off," Ablin said.

Despite a sluggish recovery and a suggestion by some analysts that the U.S. economy may already be in recession, the S&P 500 hit its highest level in 2-1/2 months last week.

The MSCI world equity index was flat on Wednesday, but has fallen 2.4 percent so far this week as concerns about the impact of Europe's problems on growth spread across the world.

U.S. dollar-denominated Nikkei futures rose 0.1 percent in choppy trading. The pan-European FTSEurofirst 300 index closed 0.07 percent lower.

Further supporting the euro, Spain and France said that a single supervisory mechanism for the bloc's banks needs to be adopted by the end of the year.

ECONOMIES SAG

The gains in the euro came despite the weak economic data from Germany and the UK, which reinforced the view that even Europe's biggest economies were being damaged by the debt crisis.

German business sentiment dropped in July for the third straight month to its lowest level in over two years, according to the latest survey by the Munich-based Ifo think tank.

British economic output also shrank much more than expected in the second quarter, hit by the euro zone debt crisis and government austerity, official data showed.

Greece was also back in the headlines with inspectors from the EU, ECB and International Monetary Fund in Athens to decide whether to keep it hooked up to a 130 billion euro lifeline or let it face default.

Three EU officials have said the team was likely to conclude Greece cannot repay what it owes, making a further debt restructuring necessary, but no decision is expected until at least September.

The benchmark 10-year U.S. Treasury note was down 3/32 in price, with the yield at 1.4008 percent after earlier rising as high as 1.44 percent. The yield hit an all-time low on Tuesday.

Gold futures rose more than 2 percent as expectations of U.S. and European monetary stimulus strengthened its appeal as an inflation hedge. Gold futures were last up 2 percent at $1,608 an ounce.

This blog has been much more focused on the joys/frustrations of parenting and super-cute photos of Caleb recently, but there will always be the element of grief and loss, as losing Micah and Judah is a huge part of my journey toward fatherhood.

I knew Ashley from seminary, and I so appreciated this post of hers, in which she shares a list of things that you should never to say to grieving parents. Ever. Ever. Got that? Well, you may still say them?but remember this list, and try as hard as you can not to share sentiments similar to any of these:

• ?You can always have another one.?
• ?Only the good die young.?
• ?God needed him/her more.?
• ?I know how you feel.?
• ?He/She was only on loan anyway.?
• ?God needed another angel.?
• ?At least you were able to have a child. Some people can?t.?
• ?This is all part of God?s Divine plan for you.?

Read her post here, as she elaborates on why all of these are horrible things to say to grieving parents. As she shares in her post, what people need is simply to know you?re there for them, that you care, that you love them, and that you?ll be there to listen whenever they decide they?re ready to talk. For me, getting cards and flowers (things that, normally, I don?t really care for and sometimes don?t see the point in getting) was one of the biggest things. And I don?t really know what it was. But the sheer volume of cards we received, and the flowers that brightened up our home as we grieved?that was one of the biggest things for me. In addition to the generosity of friends; one couple had a vacation home they offered to us for a long-weekend?which was such a blessing, to just get away from it all for awhile.

Anyway ? all this is to say that there are things you can do?but really, there isn?t much you can say. Just be with the person.

BEIRUT (AP) ? Syrian troops clashed with rebels in the city of Aleppo for a second day Saturday, forcing inhabitants to flee to safer areas in some of the fiercest fighting to date in the heart of the country's northern commercial hub, activists said.

Until recently, Aleppo, Syria's largest city, has remained largely loyal to President Bashar Assad and has been spared the kind of daily bloodshed that has plagued other cities over the course of the uprising against him.

The fighting in the Salaheddine district in the city center suggested Assad was losing his grip on one more traditional bastion of support.

"This night was very bad, there were huge explosions and the gunfire didn't stop for several hours," said Aleppo-based activist Mohammad Saeed via Skype.

He said dozens of fighters from the rag-tag Free Syrian Army entered Aleppo from the countryside and were now fighting regime troops from inside.

The clashes began Friday and continued throughout the night until Saturday morning, most of it in Salaheddine.

"The uprising has finally reached Aleppo," Saeed said.

This week, fierce fighting between troops and rebels reached the Syrian capital, the central bastion of Assad's rule, shattering parts of the city and sending thousands of people fleeing to neighboring Lebanon and Iraq.

Activists and residents reported a tense calm in Damascus Saturday but said sporadic gunfire and explosions could be heard throughout the night.

Two residents who did not want to be identified for safety reasons said by telephone that the fighting peaked between 1 a.m. and 3 a.m. local time.

Damascus and Aleppo are both home to elites who have benefited from close ties to Assad's regime, as well as merchant classes and minority groups who worry their status will suffer if Assad falls.

But for months, rebels have been gaining strength in poorer towns and cities in the Aleppo countryside, gaining footholds near the Turkish border. As an Area Sales Manager you will be responsible for generating profitable business that meets territory objectives through meeting and exceeding established sales budgets/targets and through developing corporate business within the territory to use our clients' products and services; locally; nationally and/or globally. This role will involve elements of both new business sales generation and strategic account management.
